AMC Clinical Exam 2026: Format, Stations & Preparation Strategy

If you're an International Medical Graduate (IMG) working toward registration in Australia, the AMC Clinical Exam is the final major hurdle after the MCQ (CAT). It's demanding, expensive, and unforgiving of poor structure — but it's also very learnable once you understand exactly what you're walking into. Here's what the 2026 exam looks like, station by station, and how to prepare for it without wasting months on the wrong kind of study.

What the AMC Clinical Exam Actually Tests

The Clinical Exam is an Objective Structured Clinical Examination (OSCE) that assesses whether you can practise safely at the level of a final-year medical student entering internship in Australia. It isn't a knowledge test — you already proved your theoretical knowledge by passing the MCQ. This exam is a performance test: can you take a focused history, examine a patient, counsel someone, interpret data, or manage a scenario safely, all under time pressure and in a communication style Australian examiners recognise as safe and patient-centred?

The 2026 Format at a Glance

• 20 stations total in a fixed circuit: 16 assessed stations plus 4 rest stations.
• Of the 16 assessed stations, 2 are pilot stations — new or trial scenarios the AMC is testing. Marks from these typically aren't counted toward your result while data is collected.
• That leaves 14 scored stations, and your result comes down to those.
• Pass mark: 9 out of 14 scored stations (reduced from the previous requirement of 10, following an AMC review).
• Each station runs for 10 minutes: 2 minutes of reading time outside the door, followed by 8 minutes inside to complete the task. The bell ends the station immediately, whether or not you've finished.
• The circuit runs over roughly 3–4 hours in a single sitting.
• Content spans multiple disciplines — medicine, surgery, women's health, paediatrics, psychiatry/mental health, and general practice — with stations built around history-taking, physical examination, communication and counselling, data interpretation, and procedural or management tasks.
• Each station is scored independently against a structured marking rubric by an examiner, using a borderline regression method to set the pass standard.

Where and How It's Delivered

• In-person sittings are held at the AMC National Test Centre in Melbourne.
• An online Clinical Exam, delivered via a secure, invigilated video platform, remains part of the AMC's permanent assessment offering. Online places are generally prioritised for candidates in rural and remote (MMM 2–7) areas who would otherwise struggle to travel.
• Fees: the in-person exam sits at roughly $3,000 AUD; the online version carries an additional levy (bringing it to around $3,400 AUD) to cover technology and remote invigilation costs.
• Station appeals are possible but costly (roughly $900+ per station) and rarely succeed — which is one more reason to invest your time and money in preparation rather than a post-exam review.
• Places for popular sitting blocks can fill within minutes of release, so it pays to have your AMC portal account, payment details, and documentation ready well before applications open.

Exam dates, fees, and scheduling windows change throughout the year, so always confirm current details directly on the AMC website before booking.

Why Candidates Fail — And It's Rarely Knowledge

Pass rates for the Clinical Exam sit well below those of the MCQ, and the gap isn't explained by weaker clinical knowledge. The recurring failure points are:

1. Poor time management — running out of time before reaching a safety-net or management plan.
2. Missing structure — jumping between topics instead of following a systematic approach (history → examination → differential → management → safety-netting).
3. Weak communication fundamentals — not exploring Ideas, Concerns, and Expectations (ICE), missing patient cues, or failing to build rapport in the Australian consultation style.
4. Inconsistent performance — scoring very well in some stations and very poorly in others. Examiners reward consistent, safe competence across all domains far more than occasional brilliance.

The practical implication: it's better to be reliably solid at every station than exceptional at a few.

A Preparation Strategy That Actually Works

1. Understand the rhythm before you drill content

Practise inside the real time structure — 2 minutes to read and plan, 8 minutes to execute — long before exam day. Most candidates underperform not from lack of knowledge, but from unfamiliarity with the pressure of that rhythm.

2. Build a repeatable station template

Have a default structure you can apply to almost any station: greet and orient, explore ICE, gather focused history or examination findings, synthesise a working diagnosis, explain management in plain language, and safety-net before the bell. A repeatable skeleton frees up mental bandwidth to focus on the specific clinical content.

3. Practise out loud, not just on paper

Reading textbooks builds knowledge, not performance. The exam rewards spoken, real-time communication — so the highest-value preparation is timed mock stations, ideally recorded, with structured feedback on both content and delivery.

4. Rotate through all major domains

Don't over-prepare for medicine and surgery at the expense of women's health, paediatrics, and mental health stations — the exam draws across all of them, and consistency across domains is exactly what the scoring rewards.

5. Use a realistic timeline

Most candidates benefit from around 12 weeks of structured preparation, combining content review with a high volume of timed practice stations (aim for 30+ recorded mock stations if possible) rather than months of passive reading.

6. Rehearse safety-netting and closure

With only 8 minutes per station, many candidates run out of time before telling the patient what to do next or flagging red flags to watch for. Practise leaving the final 60–90 seconds specifically for closure, so it becomes automatic under pressure.

Final Thoughts

The AMC Clinical Exam rewards structure, communication, and consistency far more than raw clinical knowledge. If you've passed the MCQ, you already have the knowledge base — the work now is translating it into a safe, repeatable, time-bound performance that an examiner can recognise in eight minutes flat. Build your template, rehearse it out loud under real time pressure, and treat every domain — not just your strongest one — as exam-critical.
